I want my ex back after splitting up from her

I split up from my ex going on two weeks now and I desperately want her back. I've tried talking to her but she tells me to back off and the other night I couldn't take it and threatened to kill myself and also have self harmed myself in a vain attempt to get her back. (I am getting professional help). But the problem now is that she won't speak to me at all and when I tried to go round I had to speak to her mother because she wouldn't see me. I feel so awful about everything and just want her back in my arms.
It's not the first time we have split up either, she split up from me nearly a year ago then we got back together, then I split up from her in november and we managed to stay an item after that but now I fear I have ruined the whole thing because I acted out selfishly and all I want to do is talk to her because I love her so much.
I feel like I'm to blame for a lot of the problems in the relationship but I know my faults and I jumped to conclusions when I split up from her because she has threatened to split from me in the past on many occasions because of the way I am and when I say it, it was what ended it for real and now I feel like shit and I can't stand not being able to speak to her,
what can I do to get her back?

I don't think you two should get back together, honestly. There is too much water under the bridge, and you have tried and tried together, on and off, for a very long time now. I know you want her in your life, but a lot of that is because it's a comfortable habit. In the long run, you will be better off with a new love. Work towards that. I am very glad you are seeking professional help. I hope you are doing well!
